I understand that all theatermakers are required to be fully vaccinated against Covid-19 in order to participate in all UPSTI programming. I agree to thoroughly review and follow the rules and precautions mapped out in the UPSTI Covid Safety and Wellness Plan that will be included in the Parent Packet.

I understand that my child will be asked to send a brief audition/interview video that will include singing(performers) and/or answering a few questions about theater. This video will not be shared or made public, and serves simply to help the Artistic and Production Team with casting the show and planning specific curriculum. Further instructions will be sent once this Registration Form has been processed. *
